Even before children learn to talk, they understand a lot more than what they can speak. As they continue to develop their communication and language skills, they begin to put their thoughts and feelings into words. But in some cases, a child may find it difficult to find the words to express themselves and have trouble speaking with others. If a child is having significant expressive language issues compared to peers, this is known as an expressive language disorder (also known as spoken language disorder) or an expressive language delay (for children 4 years and younger). 

Even before children learn to talk, they understand far more than they can say. As communication skills develop, children begin putting thoughts, needs, and feelings into words. But some children have significant difficulty finding the words to express themselves and communicating clearly with others. When a child has marked difficulty using spoken language compared with peers, this may be described as an expressive language disorder (also called a spoken language disorder) or an expressive language delay for children age 4 and younger.

One important way children express themselves is through narratives. Narrative skills help children retell stories, describe experiences, and explain past events. These skills are closely tied to school readiness, classroom participation, literacy, and long-term academic success.

Typically developing children acquire most grammatical morphemes by age four. For children who struggle with narrative language, however, parents may notice delays or gaps in storytelling, sentence structure, and overall language organization. Narrative Development in Children

Narrative development plays a major role in later literacy and academic success.

Narrative language allows a child to communicate beyond the immediate moment. When children retell an event or explain a story, they show how well they understand sequencing, vocabulary, grammar, and perspective-taking.

A child’s narrative skills reflect the ability to tell or communicate a story clearly. As these skills grow, children learn the basic rules of storytelling: sequencing events, organizing ideas, introducing characters, establishing a main idea, and considering another person’s perspective.

Narrative skills often begin developing in early childhood through shared reading, storytelling, and bedtime routines. By listening to stories and participating in conversations, children begin to internalize story structure. Many children with language impairments, however, struggle to understand, organize, and express narratives. What is an Expressive Language Disorder?

Unlike speech sound disorders, which affect how spoken sounds are produced, language disorders affect how spoken language is used and organized compared with peers. Expressive language difficulties may show up in spoken vocabulary, grammar, sentence complexity, and pragmatic language. These challenges often become more noticeable after age 4, especially when children have trouble telling stories, answering questions, or connecting socially with peers. Expressive language disorders are generally discussed in two broad categories:

Mixed Receptive-Expressive Language Disorder – The child has difficulty understanding spoken language and also has difficulty expressing thoughts clearly.

Expressive Language Disorder – Receptive language skills are relatively age appropriate, but there are significant challenges with verbal expression compared to peers.

There is no such thing as just a receptive language disorder. If a child is misdiagnosed with this label, either the child has significant attentional difficulties or the child’s expressive language skills were not thoroughly assessed. Spoken or expressive language disorder is a lifelong condition that appears in early childhood. It is often developmental in nature, though it may also occur after traumatic brain injury. A child with expressive language disorder may understand language relatively well but still struggle with written and/or verbal expression. These challenges can affect academic performance, classroom participation, and social interaction.

Causes of an Expressive Language Disorder

Expressive language disorder can be developmental or associated with other underlying conditions.

The exact cause of expressive language disorder is not always known. In some children, it is a primary difficulty. In others, it may be related to another diagnosis or medical history. Common associated conditions include:

  • developmental disorders such as autism
  • brain injury or tumor
  • birth defects (Down syndrome, cerebral palsy)
  • pregnancy or birth problems due to poor nutrition, fetal alcohol syndrome, premature birth, or low birth weight
  • hearing loss caused by ongoing ear infections
  • genetics or family history

What Are the Signs of an Expressive Language Disorder?

Depending on a child’s age, language development, and the language domains affected, the signs of an expressive language disorder can vary. These symptoms may affect one or more of the following areas:

Phonology – The ability to recognize and work with sounds in spoken language, such as rhyming or playing with sounds.

Syntax – Another word for grammar.

Morphology – A branch of grammar that focuses on the meaningful parts of words, including prefixes and suffixes.

Semantics – Vocabulary and word meaning.

Pragmatics – Using appropriate verbal and nonverbal language in social situations and daily interactions.

In many cases, the signs of expressive language disorder are subtle and are not immediately recognized by parents or teachers. Children with expressive language challenges may:

  • have a less developed vocabulary than their peers
  • frequently use fillers such as “um,” “uh,” or “huh”
  • understand more than they can say, while struggling to ask or answer questions
  • use short or repetitive phrases and sentences
  • struggle with telling stories clearly
  • for toddlers, rely heavily on gestures
  • show limited intonation or modulation while speaking
  • avoid conversation, group interaction, or social situations
  • talk a lot without communicating ideas clearly

Because expressing thoughts, feelings, and ideas can be so difficult, these challenges may also affect self-esteem and confidence. In school, children may find it hard to participate in class, answer questions, connect with teachers and classmates, or complete written work.

Diagnosing an Expressive Language Disorder

A thorough speech-language evaluation is essential for an accurate diagnosis.

As with any speech or language concern, it is important to first rule out hearing issues that may affect language development. A hearing assessment by an audiologist helps identify whether hearing loss or chronic ear infections may be contributing to the child’s communication difficulties.

To diagnose an expressive language disorder, a speech-language pathologist (SLP) must complete a comprehensive evaluation. A comprehensive assessment should include input from parents, caregivers, teachers, and other professionals when appropriate. The evaluation process usually includes both formal and informal measures. Formal assessment may involve standardized testing, while informal assessment may include interviews, observations, checklists, and language samples.

While both types of assessment matter, language sampling often provides the clearest picture of how a child communicates in real-life settings. It helps an SLP understand a child’s strengths and weaknesses across syntax, semantics, morphology, and pragmatics.

The SLPs we work with at Brooklyn Letters conduct language sampling to help assess expressive language needs, including analysis with the Systematic Analysis of Language Transcripts or SALT program. The typical expressive language development of young children, from 12 months to 47 months and beyond, is outlined in the Acquisition of Sentence Forms Within Brown’s Stages of Development. Recommendations that a speech pathologist may make:

  • Hearing test by an audiologist
  • Test for learning difficulties (for school-aged children) by a neuropsychologist

During the evaluation phase, speech language pathologists will want to understand the child’s:

  • full case history (including birth and medical records; family history of language, speech, reading, or academic difficulties; languages or dialects spoken at home; and family and teacher observations and concerns)
  • spoken language skills (phonology and phonological awareness, semantics, morphology, syntax, and pragmatics)
  • level of reading and writing, if the child is school-aged

  • Addressing fluency and managing atypical stuttering issues
  • Managing challenges related to feeding and swallowing

How to Treat an Expressive Language Disorder

Speech-language therapy helps children build clearer, more functional communication over time.

Once a diagnosis is made, the SLP will complete further analysis and observation before designing an individualized treatment plan. Therapy does not provide a permanent “cure,” but it can give children effective strategies and support to improve communication, participation, and confidence.

Treatment methods vary based on the child’s age, profile, and goals. One common technique is modeling, in which the therapist demonstrates and reinforces targeted language skills such as sounds, vocabulary, grammar, and sentence structure.

Some areas that SLPs address are:

For preschoolers (ages 3 to 5):

  • enhancing phonological awareness through rhyming, blending, and segmenting spoken words
  • improving vocabulary and understanding of semantic relationships
  • increasing sentence types, length, and complexity
  • improving conversational skills
  • developing narrative skills
  • increasing language flexibility in different contexts
  • building and encouraging literacy skills

For elementary school children (ages 5 to 10):
