Oral motor speech therapy is often debated among speech therapists, but the research community is largely aligned on the science. The term oral motor itself is ambiguous and often misunderstood. In modern speech-language pathology, the correct diagnostic terminology is speech sound disorders, which includes articulation disorders and phonological disorders.

There is an important distinction between speech oral-motor therapy and non-speech oral motor therapy. Speech oral-motor therapy—therapy that directly targets speech sounds—is supported by research. Non-speech oral motor therapy, which focuses on activities unrelated to actual speech production, lacks scientific evidence demonstrating that it improves speech.

Many speech therapists learn non-speech oral motor techniques through post-graduate workshops and professional trainings where trends sometimes take precedence over research. Importantly, the content of these trainings is not endorsed by the American Speech Hearing Association (ASHA).

Research conducted by Dr. Gregg Lof suggests that approximately 85% of speech therapists report using non-speech oral motor exercises when assessing or treating speech problems. Unfortunately, this reflects a broader issue within the field: many clinicians receive limited post-graduate support in interpreting research and applying evidence-based practices.

As a result, some children are misdiagnosed and may spend valuable therapy time working on interventions that do not meaningfully improve speech production. Effective speech therapy for speech sound disorders always includes the production or shaping of actual speech sounds.

For example, if a child is learning the /m/ sound, a clinician may guide the child to close their lips and produce “mmm.” If a child omits the final /p/ in words, therapy focuses on producing that sound in short words such as “hop,” “top,” and “mop.” This approach directly activates the neurological systems involved in speech: motor planning, coordination, and language processing.

Non-speech oral motor exercises, by contrast, involve activities such as blowing whistles, sucking through straws, or chewing on therapy tubes. These activities may strengthen oral muscles or influence general oral tone, but there is no scientific evidence demonstrating that they improve speech production.

The mouth is used for many functions including breathing, chewing, swallowing, and drinking. However, speech is neurologically unique. Talking requires minimal muscle strength compared to activities like chewing. Speech learning relies primarily on motor planning, cognitive processing, and linguistic systems—all of which are controlled by the brain.

Speech is special. Humans can produce complex language because of our neurological systems, not simply because of the strength of our mouth muscles. Other animals vocalize but cannot produce language in the same way because the differences lie in brain development, not oral strength.


If your child has difficulty controlling mouth movements for speech or eating—such as challenges with chewing, blowing, or producing specific sounds—an evaluation by a speech-language pathologist may determine whether an oral motor or speech sound disorder is present.

A useful analogy is learning to play the piano. Practicing finger strength exercises alone does not make someone a better pianist. Without actually playing the piano, stronger fingers will not produce better music. In the same way, non-speech oral motor exercises do not teach children how to produce speech sounds.

Speech scientists widely recommend that therapy focus on techniques that help children learn to produce and shape the sounds of their language. Activities that directly target speech production allow children to develop the motor planning, coordination, and linguistic awareness necessary for clear communication.

To quote an excerpt from a Letter to the Editor in the New York Times (1/24/12), the following observation highlights a broader issue that affects many healthcare professions:

“We frequently receive manuscripts in which people promote ideas and theories without adequate data, usually in pursuit of personal fame or monetary gain. Although the peer-reviewed medical literature is not without flaws, there is no better way than peer review to vet information before it is used in the diagnosis and treatment of disease. At least in medicine, we should think carefully before we decide that more, unfiltered information is better information.”
