Unlike other speech disorders that might involve muscle weakness or language difficulties, apraxia of speech primarily stems from problems in the brain’s ability to coordinate the necessary muscle movements for speech. 

Apraxia of speech can affect people of all ages, but it is most commonly diagnosed in children. Early intervention with speech therapy is crucial for individuals with this disorder. Speech-language pathologists work with individuals with apraxia to improve their speech sound production and overall communication skills through specific therapy techniques and strategies tailored to their needs.

Symptoms of Speech Apraxia

Several distinct symptoms characterize Apraxia of speech:

  1. Inconsistent Speech Errors: Individuals with apraxia of speech typically exhibit inconsistent errors when trying to produce speech sounds. They may have difficulty repeating the same word or sound correctly, even in conversation.
  2. Difficulty Planning Speech Movements: Apraxia of speech is rooted in difficulties with motor planning and coordination. Individuals need help planning the precise muscle movements required to produce speech sounds.
  3. Sound Omissions and Additions: People with apraxia may omit sounds from words, add extra sounds, or use incorrect sounds in their speech. These errors often make their speech difficult to understand.
  4. Limited Sound Repertoire: Individuals with apraxia may have limited speech sounds they can produce accurately. This can result in speech that sounds immature or unclear.
  5. Struggle with Multisyllabic Words: Longer or more complex words, especially those with multiple syllables, can be particularly challenging for individuals with apraxia. They may need help to articulate these words correctly.
  6. Inconsistent Stress Patterns: Stress patterns within words may be inconsistent, making speech sound rhythmically irregular.
  7. Groping Movements: Sometimes, individuals with apraxia of speech may appear to make “groping” movements with their articulators (lips, tongue, etc.) as they attempt to produce specific sounds or words.

Co-Occurring Characteristics and Symptoms of CAS

Children with Childhood Apraxia of Speech (CAS) often face additional challenges that may affect expressive language and literacy development (Lewis et al., 2004; McNeill, Gillon, & Dodd, 2009b). These include:

Sensory and Motor Co-Occurrence (Crary & Anderson, 1991; Davis et al., 1998; Dewey, Roy, Square-Storer, & Hayden, 1988; McCabe, Rosenthal, & McLeod, 1998; Shriberg et al., 1997):

  • Gross and fine motor delays
  • Motor clumsiness
  • Oral and limb apraxia

  • Abnormal oral sensory perception (hyper- or hyposensitivity)

Causes of Speech Apraxia

While its precise cause varies among individuals, it is fundamentally rooted in neurological factors. Genetic predisposition, developmental delays, brain injuries, and other neurodevelopmental conditions are all contributing factors. 

Environmental factors, such as toxins or infections during pregnancy, can also play a role. It’s crucial to note that emotional or psychological factors are generally not the cause of apraxia.

  • Targets phonological pattern errors in highly unintelligible speech with omissions, substitutions, and limited consonant use.
  • Treatment occurs in 5 to 16-week cycles, addressing one or more phonological patterns. New cycles target additional patterns until they naturally appear in the child’s spontaneous speech (Hodson, 2010; Prezas & Hodson, 2010).
  • Goal: Stimulate sound/pattern emergence, not achieve phoneme mastery, mimicking typical phonological development.

Integrated Phonological Awareness (IPA):

  • Designed for preschool and young school-age children to enhance phonological awareness, letter-sound knowledge, and speech production.
  • Strategies include:
    • Teaching activities like nursery rhymes to develop sound awareness.
    • Integrating phoneme awareness games and letter activities during treatment (McNeill, Gillon, & Dodd, 2009a, 2010; Moriarty & Gillon, 2006).

These approaches strengthen linguistic and phonological skills, supporting overall speech development.

Childhood Apraxia of Speech (CAS) requires a specialized, intensive intervention approach because the disorder affects the brain’s ability to coordinate speech movements. Unlike traditional speech therapy that targets sound production, CAS therapy focuses on motor learning and movement sequences.

Key Treatment Approaches for CAS

Speech therapy for CAS is typically intensive, requiring frequent and structured practice to help the child learn correct speech movements. Below are the most evidence-based treatments used by speech-language pathologists:

1. Dynamic Temporal and Tactile Cueing (DTTC)

  • DTTC is one of the most effective approaches for treating CAS.
  • It uses repetition, sensory cues (tactile, visual, auditory), and gradual fading of support to help children improve their speech motor planning.
